Pathways to Private, Government-funded or ACC funded care & support

Privately / Self-funded Pathway

Call us to discuss your needs and we will advise how we can tailor a support plan to suit you. We have a team of Nurse coordinators ready to handle the more complex care requirements. Information packs can be mailed to you for further consideration.

ACC Funded Pathway

If you have an injury that is covered by ACC, you may be eligible for funded support to help you around your home. To apply, head to ACC’s website and complete their “acc001′ form.  Your GP can also send this request through to ACC. An independent assessor organised by ACC, will visit you in your home. Once your application is approved, ACC will send a referral to us to, but you need to let them know, that you want Home Support North to provide this assistance. Call us if you need assistance or clarification on this process. 

Government-funded Pathway.

Follow with 5 steps below to get care and support in your home and community. You must be eligible to receive publicly funded healthcare in New Zealand to receive government-funding for your home support.

Step 1

Have a chat with us about the services we offer and how we can assist you. 

Ask us to mail you information with instructions on how to get care and support at home.

Step 2

If you want to get government-funded home help, you must have a needs assessment completed by NASC (Needs Assessment Service Coordinator). These assessments are normally arranged by your doctor if you let them know you need assistance at home.  

Step 3

A Needs Assessor (NASC) will contact you and let you know if you qualify for government funded care. They will determine the level of care that can be provided. When you speak with the Needs Assessor, tell them you would like Home Support North to be your provider.

Step 4

NASC sends a referral through to us.  Our Coordinator meets with you in your home to create a custom support plan. 

Step 5

A helpful Support Worker arrives and your services begin.

We will keep in contact with your regularly to ensure your care plan is right for you.

1. Contact Home Support North

Talk to one of our Customer Service agents to find out how we are able to work with you to establish a care plan to suit your needs. 2. An Assessment is your first step to identify how your support will be funded

For many people in Northland, Home and Community Support Services are funded by the Ministry of Health and if you qualify for assistance, you can provide support with household tasks and personal care so you can live independently at home.
If you are 65 years old or over, you will need to be assessed by the Northland Needs Assessment Service (NASC). If you are under 65 and have a disability, you can arrange an assessment with Northable in Whangarei. This assessment will identify what support you need.

3. Request Home Support North during your assessment

At your assessment, ask your Needs Assessor for Home Support North by name to ensure you get access to local services provided by professional and fully trained support workers from within your own community.

We hold an ACC sub-contract, which means we can provide a range of independent living support services to people recovering from an accident.

If you are living with a disability, you may qualify for Individualised Funding (IF). We can assist you to manage your package of care with various levels of support and options, making sure things run smoothly.

4. Building your Support Plan with your Nurse Coordinator

If you qualify for government funding or are choosing to receive privately funded supports, our qualified Nurse Coordinator will visit your home to get to know you and your needs. Together, you will discuss daily activities and create a personalised support plan that empowers you to live safely and comfortably at home.

This may include assistance with housework, meal preparation, shopping, laundry and more. We can also provide personal care which includes bathing/showering, feeding, transportation to appointments and community access.

5. Our caring Support Workers begin your cares and supports

Once your personalised support plan is in place, a dedicated Home Support North Support Worker will be assigned to you. This friendly and qualified professional will work alongside you to deliver the care outlined in your plan, helping you maintain your independence and well-being in the comfort of your own home.

Combined Funding Care & Support

If you wish, you can supplement the Home Support package funded by the government and pay privately for additional services to customise a package of cares that suits your needs. Please contact us if you would like to discuss your options.
